About the role
ALIYA Healthcare is seeking an experienced Corporate Director of Social Services to lead Social Services departments across our skilled nursing facilities.
Responsibilities
- Provide leadership, guidance, and support to Social Services departments across multiple skilled nursing facilities
- Develop and implement programs, policies, and procedures that promote resident well-being and regulatory compliance
- Conduct routine facility visits to assess Social Services operations and identify opportunities for improvement
- Mentor, train, and develop facility Social Services Directors and team members
- Ensure compliance with federal, state, and local regulations governing Social Services in long-term care settings
- Aid facilities with survey preparation, survey response plans, and regulatory compliance initiatives
- Monitor resident satisfaction, grievance resolution, discharge planning, and psychosocial outcomes
- Collaborate with Administrators, Directors of Nursing, and interdisciplinary teams to improve resident care and quality measures
- Support facilities with admissions, transitions of care, discharge planning, and complex resident situations
- Review documentation practices to ensure accuracy, timeliness, and compliance
- Aid with recruitment, onboarding, and performance management of Social Services personnel
- Participate in quality assurance and performance improvement initiatives
- Develop and track departmental metrics and performance indicators
- Promote a culture of resident-centered care, accountability, and continuous improvement
- Perform other duties as assigned
Qualifications
- Bachelor's Degree in Social Work, Human Services, Psychology, or a related field required
- Master's Degree preferred
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred
- Minimum of 5 years of Social Services leadership experience in skilled nursing, long-term care, or post-acute care
- Multi-site or regional leadership experience preferred
- Strong knowledge of CMS regulations, discharge planning, resident rights, behavioral health, and long-term care operations
- Proven leadership, coaching, and team development skills
- Excellent communication, organizational, and problem-solving abilities
- Ability to travel regularly to assigned facilities
