Essential Duties
- Manage and direct the Social Services Department
- Identify and provide for residents' social, emotional, and psychological needs
- Accurately assess, document, and provide interventions to meet psychosocial needs of all residents
- Develop care plans based on assessments
- Maintain a quality working relationship with the medical profession and other health-related facilities and organizations

Preferred Qualifications
- Two years of job-related experience, preferably in long-term care
- Bachelor’s degree in social work or related field preferred
- Experience with social work, preferably working directly with patients and their family members at a nursing facility
- Detail oriented, computer literate, and possesses excellent communication skills
