Coordinator of Adolescent Day Therapy

Assesses, plans, implements, and evaluates nursing care for patients in Child/Adolescent Day Treatment programs. Provides assessment and therapy for patients and their families. Coordinates the patient’s plan of care and daily schedule with the treatment team and communicates with the patient’s external support system.

Schedule

Alternating two-week schedule:

  • Week 1: Monday (7:00 AM–3:30 PM), Tuesday (2:00 PM–10:30 PM), Friday (7:00 AM–3:30 PM), Saturday (7:00 AM–3:30 PM), Sunday (7:00 AM–3:30 PM)
  • Week 2: Wednesday (2:00 PM–10:30 PM), Thursday (2:00 PM–10:30 PM)

Responsibilities

  • Provides clinical and program services to patients and their families.
  • Delivers timely treatment and appropriate responsiveness to immediate therapeutic needs.
  • Facilitates psycho-educational therapy groups, individual therapy, or patient support groups.
  • Initiates day-to-day implementation of the program schedule and coordinates the multi-disciplinary team.
  • Communicates clinical or medical concerns to the interdisciplinary team, physician, Program Manager, and/or Clinical Systems Supervisor.
  • Assesses patient care needs and identifies expected nursing outcomes.
  • Involves the patient, family, and other multi-disciplinary team members in gathering data.
  • Performs admission interviews and health and social history assessments.
  • Plans and implements the nursing care plan, modifying and evaluating as needed.
  • Documents data and identified outcomes.
  • Correctly administers medication, adheres to medication policies, and is knowledgeable of actions, contradictions, and potential side effects.
  • Observes and documents medication outcomes.
  • Facilitates self-administration medication programs as directed.
  • Contributes to and monitors the therapeutic treatment plan.
  • Collaborates with the patient, family, and multi-disciplinary team to develop a therapeutic treatment plan.
  • Identifies outcomes that are age-appropriate, culturally appropriate, realistic, and attainable.
  • Documents the treatment plan to ensure continuity throughout the continuum of care.
  • Plans and facilitates discharge.
  • Advances evidence-based practice by utilizing standards, guidelines, and pathways for care delivery.
  • Incorporates data and information into practice to continually improve care and enhance patient outcomes.
  • Promotes a culture of evidence-based practice through activities such as:
    • Utilizing evidence-based policies in clinical practice.
    • Attending and participating in learning opportunities about evidence-based practice.
    • Membership in hospital or unit-based nursing practice councils or research councils.
    • Participation in evidence-based practice projects (e.g., policy development or poster presentations).
    • Engagement in journal clubs, journal article critiques, literature reviews, or development of standards/pathways/guidelines for patient care.
    • Delivering presentations or conducting research studies.
  • Provides care or service adjusting approaches to reflect the developmental level and cultural differences of the population served.
  • Partners with patient caregivers in care and decision-making.
  • Communicates in a respectful manner and ensures a safe, secure environment.
  • Individualizes the plan of care to meet patient needs.
  • Modifies clinical interventions based on the population served.
  • Provides patient education based on an assessment of learning needs of the patient/caregiver.
  • Fulfills all organizational requirements, including completing required learning relevant to the role.
  • Complies with and maintains knowledge of all relevant laws, regulations, policies, procedures, and standards.
  • Fosters a culture of improvement, efficiency, and innovative thinking.
  • Performs other duties as assigned.

Requirements

  • Current Minnesota Registered Nurse (RN) license.
  • Basic Life Support (BLS) certification.

Preferred Qualifications

  • Bachelor’s Degree.

Pay

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. The organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of the offer.
