Community Support Supervisor
Job Summary
As a Community Support Supervisor, you will play a pivotal role in ensuring the efficient operation of Community Support services to our Health Plan members. You will be responsible for providing supervision, managing daily and weekly reports, coordinating with health plans for referrals and authorizations, overseeing staff assessments, and ensuring housing deposit processes are completed accurately and with proper documentation. Your role will involve effective communication with various partners, including health plans, landlords and members.
Supervisory Responsibilities
- Provide leadership, guidance, and support to CS staff.
- Generate and analyze daily and weekly reports for member progress.
- Maintain and update the Master Log daily.
- Review Ring Central Analytics, productivity logs, and member notes.
- Oversee staff notes and authorization processes in Alliance.
- Collaborate with health plans for referrals and authorizations, ensuring compliance.
- Stay informed on health plan policies and procedures.
- Clock in and out of the Health Plan Housing Goods Form.
- Ensure quality and consistency in staff assessments.
- Monitor assessment processes for regulatory adherence.
- Clock in and out of housing deposit processes, verifying accuracy with landlords.
- Communicate effectively with stakeholders, maintaining accurate documentation and promptly addressing inquiries and requests.
Education and Experience
- A high school diploma or GED is required.
- A bachelor’s degree is preferred.
- A bachelor's degree in social work or healthcare administration, or at least 4 years of related work experience is required.
- Previous experience in a supervisory role, preferably in community support services or healthcare is preferred.
Required Skills/Abilities
- Bilingual (English/Spanish) is preferred.
- Strong leadership and communication skills are required.
- Knowledge of health plan requirements and authorization processes is essential.
- Experience with data analysis and reporting is beneficial.
- The ability to multitask and prioritize responsibilities effectively is necessary.
- An attention to detail and accuracy in documentation and record-keeping is crucial.
Physical Requirements
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 10 pounds at times.
Pay Range
$29 USD - $32.50 USD
