Position Summary The Community Resource Navigator for the Creando Puentes program (Navigator) conducts outreach to the Latino community with the goal of assisting individuals with symptoms of mental illness to connect with needed services. They provide education and case management services to individuals with symptoms of mental illness in English and Spanish. The Navigator serves as a liaison between clients, CLUES, and Ramsey County Human Services through the Creando Puentes Program. Department: Family Services Reports to: Associate Director of Community Resource Programs Location: CLUES St. Paul, MN Office Position Type: Regular, Non-exempt Schedule: Full-Time. Generally, Monday – Friday Office hours. Estimated Pay: $22.50 to $24.50 (depending on experience & qualifications). Essential Duties And Responsibilities Conduct outreach and engagement activities: participate as part of a team to provide outreach to the Latino community with the goal of assisting individuals with symptoms of mental illness to connect with needed services. Serve as a liaison between individuals, CLUES, and Ramsey County Human Services. Reach out to individuals in a variety of community locations. Provide case management: identify needed services, assist individuals in accessing services (including transportation), advocate on behalf of individuals served to ensure that needs are met. Visit individuals in their homes. Work within the context of the client and his/her family.Provide education regarding mental illness and services available to individuals, families, or groups. Provide crisis intervention and/or plans for clients and their families. Maintain up-to-date client records, including case notes. Coordinate data collection and program reporting.Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards.Interact with multi-disciplinary team (clinical and social work professionals) to support the clients Provide input and feedback to refine the Creando Puentes Program, the development of the team, and the processes involved.Cultivate working relationships with service collaborators, and other appropriate stakeholders.Assist in the delivery of contracts outcomes and relationships with contract stakeholders. Participate in all internal and external activities as required by program contracts.Participate in program meetings, regular department meetings; agency “All Staff” meetings, and other meetings as assigned.Promote the mission, operating values and agency services of CLUES within the community.Carry out all other duties as assigned by supervisor. SUPERVISORY RELATIONSHIPS This position has no supervisory responsibilities. Qualifications Education, Experience & Key Abilities Bachelor’s degree in social work, Psychology, Counseling, Public Health or related field or equivalent combination of education and experience required.Experience in providing case management services preferred. Experience providing outreach/engagement services in the community preferred. Experience working with individuals with symptoms of mental illness preferred.Bilingual in Spanish & English with cultural sensitivity and awareness required. Experience/involvement with and understanding of Latino community required. Strong communications skills and ability to establish positive working relationships with clients/families, health care providers, and other involved community members required.Ability to work independently and as a member of a team required. Must be able to maintain a flexible schedule – generally Monday through Friday office hours, but some evenings and weekends may be necessary. Must be willing and able to transport clients in personal vehicle while maintaining valid driver’s license, proof of automotive insurance, and maintain a good driving record. Language Skills Effective communication in English and Spanish, both verbally and in writing required.Ability to read, analyze, and interpret written materials, maintain accurate documentation, and respond appropriately to questions and requests. Transportation Must have a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must be willing and able to safely transport clients in personal motor vehicle.Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check. Reasoning Ability Ability to solve practical problems and interpret instructions in various forms. Computer Skills Proficient in Microsoft Office (Word, Excel, PowerPoint) and other common software (e.g., database, reporting, client management systems, etc.). Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties. WORKING CONDITIONS Background check Must pass a criminal background check, driving record check, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these, and additional background checks.
