Provides operational support and transition duties for identified highest-risk complex patients. Works collaboratively with the care management team, patients, family caregivers, healthcare providers, payers, community-based providers, and other involved parties to deliver effective, efficient, and patient-centered services.

Responsibilities

  • Understands, practices, and promotes the philosophy and guiding principles of the department.
  • Demonstrates knowledge of HIPAA regulations and maintains confidentiality of patient information.
  • Referral Management: Screens, monitors, and prioritizes referrals; maintains records of average daily caseload and workload.
  • Patient Consultation: Contacts new patients by phone to introduce the program, obtain verbal consent, and schedule initial assessments.
  • Interventions:
    • Follows up with patients to reinforce education, self-management, and care planning actions.
    • Collaborates with healthcare providers across the continuum to meet patient care planning needs.
    • Advocates for patients by communicating with providers, payers, physicians, and community-based services to establish integrated care plans.
    • Promotes mental health integration through collaboration with behavioral health providers.
  • Transition Management: Ensures effective transitions of care between healthcare settings; participates in process improvement initiatives.
  • Maintains current community-based services and provider resource lists.
  • Performs assigned duties promptly and accurately to facilitate efficient operations and communication; escalates concerns as needed.
  • Leads interdisciplinary care conferences and huddles using collaborative practice models.
  • Clerical/Support:
    • Completes timely and accurate documentation in medical records, adhering to clinical and billing standards.
    • Organizes and prioritizes daily work based on patient needs.
    • Ensures productivity standards and expectations are met.
    • Works with team members and leadership to develop standard work and best practices.
  • Clinical Support (where applicable):
    • Prioritizes patient needs via phone, electronic communication, or in-person interactions.
    • Reports significant changes in patient condition to the care team.
    • Follows provider instructions for pharmacy refills, scheduling procedures, lab result tracking, and communication tasks.
    • Maintains inventory of supplies and restocks as needed.

Requirements

  • Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
  • Certification, Associate Degree, or Bachelor's degree in a healthcare field.
  • Excellent interpersonal and communication skills.
  • Ability to adapt quickly to changing needs.
  • Knowledge of available health resources.
  • Physical demands: Lifting, twisting, standing, seeing, manual dexterity, speaking, sitting.

Preferred Qualifications

  • Bachelor's degree from an accredited institution.
  • Experience in discharge/transition planning, healthcare coaching, health advocacy, or as a medical assistant.

Schedule

  • Hybrid role requiring on-site presence for training and meetings.
  • Four 10-hour shifts per week, typically Monday through Friday between 7:00 a.m. and 5:30 p.m.
  • Hours may vary based on program and operational needs, including adjusted start/end times and occasional weekend coverage.

Pay

$20.82 - $31.67 per hour, dependent upon experience.
