About the Role: The Community & Patient Liaison is a unique hybrid role that combines traditional clinic support with referral coordination, provider relationship management, and community outreach. The goal is to create an exceptional patient and provider experience while helping grow the Cancer Center through strong local engagement. Responsibilities: Clinic Operations Prepare clinic and support oncologist during clinic daysRoom patients, obtain vitals, and assist with patient flowCoordinate labs, imaging, treatment orders, and follow-up appointments before the patient leavesSchedule infusions, office visits, and outside referralsTrack outstanding tests and ensure patients stay on their care planServe as the primary local clinical support resource Referral Coordination Obtain medical records and required documentation as necessaryEstablish and maintain external referral relationshipsFollow up on pending referrals and referral barriersCommunicate appointment status back to referring officesMaintain referral activity within the CRM/EMREnsure a seamless referral experience for providers and patients Community Provider Outreach Visit referring provider offices on a regular cadenceBuild relationships with PCPs, specialists, surgeons, hospitalists, ED providers, office managers, and referral coordinatorsEducate practices on referral processes and Cancer Center servicesDeliver updated marketing materialsProvide referral appreciation gifts (cards, chocolates, etc.)Identify opportunities and barriers to referral growth Marketing & Community Engagement Support provider lunches and educational eventsParticipate in community outreach activitiesCoordinate with central marketing on local campaigns and collateralSupport local online presence and Cancer Center awareness initiatives Performance & Reporting Track referral volume and conversionMonitor time from referral to first appointmentDocument community outreach activityIdentify referral trends and improvement opportunitiesProvide regular updates on provider engagement and growth initiatives Success Measures High patient satisfaction and efficient clinic operationsStrong relationships with community providersIncreased referral volume and reduced referral leakageTimely referral processing and schedulingConsistent provider outreach and engagement Qualifications: Certified Medical Assistant, Certified Nursing Assistant or Licensed Practical Nurse requiredClinical experience in a primary care, specialty care or ambulatory healthcare settingProficiency in using electronic health record systems for documentation, scheduling and care coordinationExperience providing patient education and presenting to community groupsAbility to support outreach, education and engagement initiatives for new clinical programs or service linesDemonstrated commitment to community engagement and access to careExcellent interpersonal and communication skills to build rapport with physicians, patients and community partnersAbility to work collaboratively with clinical and administrative teamsCompassion for delivering patient-centered care and building community health initiatives Those who thrive at Main Street Health possess these qualities: An entrepreneurial spirit.
