About the Role

At Peace Healthcare Agency is seeking an experienced, highly motivated, and relationship-driven Community Outreach & Healthcare Marketing Specialist to help expand our presence throughout Philadelphia and surrounding communities. This is an excellent opportunity for a healthcare marketing, business development, community relations, or outreach professional who knows how to build referral relationships, identify new opportunities, and turn connections into meaningful partnerships.

What You'll Do

Community Outreach & Referral Development

• Develop and maintain strong relationships with hospitals, rehabilitation centers, skilled nursing facilities, physicians, discharge planners, case managers, social workers, hospices, assisted living communities, and other healthcare organizations
• Conduct regular in-person visits, presentations, and outreach activities with referral partners
• Identify and develop new referral sources and strategic community partnerships
• Educate healthcare professionals, families, and community organizations about At Peace Healthcare Agency's services
• Represent the agency at health fairs, networking events, healthcare conferences, community events, and professional meetings
• Establish At Peace Healthcare Agency as a trusted and recognizable healthcare resource within the Philadelphia community

Healthcare Marketing & Business Development

• Develop and execute strategic marketing and outreach plans designed to increase referrals, admissions, and agency growth
• Build a strong referral pipeline and consistently work toward business development goals
• Identify opportunities to expand the agency's services and market presence
• Monitor outreach activities, referral sources, and results using CRM and agency reporting systems
• Provide leadership with feedback regarding market trends, referral opportunities, competitor activity, and community needs
• Assist with marketing campaigns, educational materials, presentations, and community initiatives

Relationship Management

• Develop long-term partnerships based on trust, responsiveness, and exceptional service
• Maintain consistent communication and follow-up with referral partners
• Serve as a professional representative of At Peace Healthcare Agency in the community
• Work closely with leadership to strengthen referral relationships and expand the agency's network
• Ensure referral partners receive timely communication and a positive experience when working with our agency

Cross-Functional Collaboration

• Collaborate closely with leadership, intake, operations, and clinical teams to support a smooth referral and admissions process
• Communicate referral opportunities and client needs accurately and efficiently
• Help ensure that potential clients and referral partners receive timely follow-up
• Support community education and outreach initiatives that promote quality, compassionate healthcare services

Required Qualifications

• Previous experience in healthcare marketing, healthcare business development, community liaison, home care sales, hospice marketing, physician relations, healthcare outreach, or a related field
• Demonstrated ability to develop and maintain professional referral relationships
• Strong communication, presentation, networking, and relationship-building skills
• Results-oriented with the ability to work toward measurable referral and growth goals
• Self-motivated, proactive, and comfortable working independently
• Professional presence and the ability to communicate effectively with healthcare executives, physicians, case managers, social workers, and other referral partners
• Strong organizational and time-management skills
• Valid driver's license and reliable transportation for local travel

Preferred Qualifications

• Experience in home health, home care, hospice, private duty, healthcare staffing, senior care, or related healthcare services
• Established professional relationships with hospitals, physicians, rehabilitation facilities, skilled nursing facilities, case managers, discharge planners, or senior care organizations in the Philadelphia area
• Existing community and healthcare referral network
• Bachelor's degree in Marketing, Business Administration, Healthcare Administration, Communications, or a related field is preferred but not required
• Experience using CRM systems, Microsoft Office, and other business development tools

Schedule

• Monday–Friday
• Local travel throughout Philadelphia and surrounding communities
• Occasional evenings or weekends may be required for networking and community events
