What You'll Do
Community Outreach & Referral Development
- Develop and maintain strong referral relationships with hospitals, rehabilitation centers, skilled nursing facilities, physicians, discharge planners, case managers, social workers, hospices, assisted living communities, senior care organizations, and other healthcare professionals throughout Bucks County.
- Conduct regular in-person visits, presentations, and outreach activities with referral partners.
- Identify and cultivate new referral sources and strategic community partnerships.
- Educate healthcare professionals, community organizations, and families about At Peace Healthcare Agency's services.
- Represent the agency at health fairs, networking events, healthcare conferences, community events, and professional meetings.
- Increase awareness of At Peace Healthcare Agency and strengthen our reputation as a trusted healthcare resource within Bucks County.
Healthcare Marketing & Business Development
- Develop and execute strategic outreach initiatives designed to increase referrals, admissions, and agency growth.
- Build and maintain a strong referral pipeline throughout Bucks County.
- Identify new business development opportunities and potential strategic partnerships.
- Work toward established referral and business development goals.
- Track outreach activities, referral sources, marketing efforts, and outcomes using CRM and agency reporting systems.
- Provide leadership with insights regarding market trends, community needs, referral opportunities, and competitive activity.
- Assist with marketing campaigns, presentations, promotional materials, and community education initiatives.
Relationship Management
- Build long-term relationships with referral partners based on trust, professionalism, responsiveness, and exceptional service.
- Maintain consistent communication and timely follow-up with referral sources.
- Serve as a professional representative of At Peace Healthcare Agency throughout Bucks County.
- Strengthen existing partnerships while continuously identifying opportunities to develop new relationships.
- Collaborate with agency leadership to increase brand awareness and community engagement.
Collaboration
- Work closely with leadership, intake, operations, and clinical teams to support an efficient referral and admissions process.
- Communicate referral opportunities and client needs accurately and promptly.
- Help ensure referral partners receive timely follow-up and excellent service.
- Support community education initiatives that promote quality, compassionate, and person-centered healthcare services.
Required Qualifications
- Previous experience in healthcare marketing, healthcare business development, community liaison, home care sales, hospice marketing, physician relations, healthcare outreach, or a related field.
- Proven ability to develop and maintain professional relationships with healthcare and community referral sources.
- Excellent communication, presentation, networking, and relationship-building skills.
- Results-oriented with the ability to work toward measurable referral and business development goals.
- Self-motivated, proactive, and comfortable working independently.
- Professional presence and the ability to communicate effectively with physicians, healthcare executives, case managers, social workers, discharge planners, and other referral partners.
- Strong organizational and time-management skills.
- Valid driver's license and reliable transportation for local travel.
Preferred Qualifications
- Experience in home health, home care, hospice, private duty, healthcare staffing, senior care, or related healthcare services.
- Established relationships with hospitals, physicians, rehabilitation facilities, skilled nursing facilities, case managers, discharge planners, assisted living communities, or senior service organizations in Bucks County.
- Existing healthcare or community referral network.
- Bachelor's degree in Marketing, Business Administration, Healthcare Administration, Communications, or a related field preferred but not required.
- Experience with CRM systems and Microsoft Office.

Position Details
Location: Bucks County, PA
Employment Type: Full-Time
Schedule: Monday–Friday
Travel: Local travel throughout Bucks County and surrounding communities
Events: Occasional evenings or weekends may be required for networking and community events
