About the role
At Swan Hospice, we are committed to providing compassionate and personalized end-of-life care to terminally ill adult patients and their families. As a Community Liaison, you will play a vital role in facilitating communication and collaboration between our organization and the community we serve.
Responsibilities
- Develop and maintain relationships with community leaders, healthcare providers, and other stakeholders to promote our organization's services and build partnerships
- Identify and address community needs, developing strategies to increase awareness and utilization of our services
- Provide education and support to patients, families, and healthcare providers on our services and resources
- Participate in community events and networks to promote our organization and enhance our reputation
- Collaborate with our care team to ensure seamless transitions and coordination of patient care
- Develop and maintain accurate records and reports to track community outreach and engagement activities
Requirements
- Bachelor's degree in a related field (e.g., social work, public health, healthcare administration)
- Prior experience in community outreach, patient engagement, or healthcare administration
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Familiarity with hospice care and the needs of terminally ill patients and their families
