Community Health Worker
About the role
A Community Health Worker contributes to AHF by creating connections between vulnerable populations and AHF healthcare center providers. The role involves extensive outreach to re-engage at-risk patients and retain them in care to achieve desirable health outcomes, regardless of payor source. As a passionate advocate for AHF’s top initiatives, you will enhance the organization’s reputation by providing high-quality, patient-facing service while keeping AHF’s Core Values and Mission at the forefront of all activities.
AHF operates in a dynamic environment with a collaborative organizational structure, where staff are accountable to multiple leaders. You will work as part of a close-knit team to keep patients engaged in their healthcare and assist in navigating and eliminating barriers that prevent successful engagement in care.
Requirements
- High school diploma or equivalency required.
- Associate or Bachelor’s degree in Public Health specializing in community health, social services, or a related field preferred.
- Three years of related experience and/or training; or an equivalent combination of education and experience.
