Community Health Worker

Educate patients

• Help patients set personal health related goals.
• Aid patients in accessing transportation for appointments.
• Navigate patients through the care continuum including appointment scheduling, visit completion, follow-up coordination and verification with the physician that the patient understood the results of the visit.

Collaborate and Report

• Collaborate on strategy and program roll-out.
• Report on barriers and successes of the program to the CHW program coordinator/manager.

Language Services

• Complete Spanish interpreter certification.
• Provide language translations at the hospital and in the community to help access medical resources.
• Aid community members in finding services such as rent relief, housing, healthy food and energy assistance.
• Aid community members with applications for FAP or Medicaid.
