What You Will Do
- Health system navigation and resource coordination
- Health promotion and coaching
- Health education
- Culturally appropriate health information and education
- Culturally relevant health education regarding social determinants of health
Essential Qualifications
- Ability to work the specified hours
- Visual acuity adequate to perform position duties
- Effective communication skills
- Ability to hear, understand, and distinguish speech and other sounds
Required Education, License/Certification, Or Work Experience
- High School or GED Equivalent
- Community Health Worker - Certification Board Certification must be completed within applicable state guidelines
- Drivers License - Licensing Board
Preferred Education, License/Certification, Or Work Experience
- Experience with navigating local medical and social support systems
