Advocate

Serve as a trusted partner and advocate by helping participants navigate healthcare services, coordinate appointments, connect with community resources, and achieve their individual health goals.

Empower

• Provide health education and wellness coaching
• Collaborate with healthcare providers and case managers
• Accompany participants to medical appointments
• Support personalized care plans that promote healthier, more independent living
• Document services accurately to meet Medicaid requirements

Requirements

• High school diploma or GED equivalency
• Must be 18 years of age or older
• Valid driver’s license with clean driving record
• May require training/testing to drive accessible vehicles
• Prior experience working with children or adults with disabilities or challenging behaviors preferred
• Successful completion and certification in training provided by agency within specific time allotment required
• Active CHW certification required at hire or must be obtained within the first 90 days as a condition of continued employment
• Certification training and associated costs are covered by LifeScape (Contingent upon acceptance into program and passing of background/fingerprint check)
