Community Health Worker

About the role

The Community Health Worker (CHW) at AHF blends the organization's mission with care coordination. The CHW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health. The CHW enhances AHF’s reputation by contributing to improved member health outcomes and advancing health equity.

Responsibilities

• Support adherence and compliance to care plans
• Identify and address socio-economic factors impacting overall health
• Advocate for AHF’s core values and mission
• Work collaboratively with a close-knit team to address various health issues
• Assist members in identifying barriers to care and connecting them with community resources
• Proactively assist members by addressing physical, emotional, and social needs
• Work with behavioral health specialists to make referrals
• Schedule appointments on behalf of members

Requirements

• Knowledge of available services
• Strong communication skills
• Bilingual Spanish proficiency preferred

Qualifications

• Experience in healthcare or related field
• Passion for public health and community outreach
• Ability to work independently and as part of a team
• Strong interpersonal and problem-solving skills
