Overview

The Community Habilitation Worker R - Enhanced 3 (CHWR-E 3) will provide skill building training and supports to individuals with developmental disabilities with high levels of needs. Supports can take place in the community and/or in a certified setting.

Primary Responsibilities

  • Mentoring / Teaching Services: Provide mentoring and teaching services to individuals with developmental disabilities to help them achieve personal outcomes and growth.
  • Daily Living Skills: Teach daily living skills such as money handling, socialization, cooking, job search, etc.
  • Other Skills: Teach any other skills as identified in the individual's plan.
  • Lifting & Hygiene: Perform lifting or assisting with hygiene functions for selected individuals.
  • Outcome Planning: Work on the individual's valued outcomes with steps on how to achieve it based on community supports and behaviors observed, along with the individual's strengths and needs.
  • Regular Meetings: Meet with the assigned individual several times per week, depending on coordinated and approved hours.
  • Travel: Travel to the specific location assigned.
  • Training: Attend and remain up to date on all required trainings, with SCIP Training being mandatory.
  • Documentation: Document each training session in the appropriate databases and report the progress and barriers with each client.
  • Supervision: Receive supervision from the Community Habilitation Supervisor.

Qualifications and Attributes

  • Prior Experience: Prior experience with related populations, such as Intellectual Developmental Disabilities and Autism Spectrum Disorders.
  • Interpersonal Skills: Strong interpersonal and engagement skills.
  • Documentation: Ability to document services and understand regulations.
  • Physical Characteristics: Must be capable of sitting or standing for long periods, moving throughout the workday, working in an open space floor plan, moving in tight spaces, and occasionally lifting more than 25 pounds.
