Responsibilities

• Conduct service needs assessments and ongoing client evaluations
• Develop, implement, and review individualized service plans
• Connect clients and families with community resources and support services
• Coordinate care with providers, families, schools, medical professionals, and community agencies
• Advocate for clients to obtain needed services and benefits
• Monitor client progress and adjust services as needed
• Assist with discharge planning and aftercare coordination
• Maintain accurate clinical documentation and case notes within required timeframes
• Participate in daily team meetings and case reviews
• Complete chart audits and ensure Medicaid compliance standards are met
• Serve as a liaison between clients, families, referral sources, and providers
• Support clients experiencing psychiatric, substance use, or crisis-related challenges
• Maintain professionalism and compassionate care in all interactions

Qualifications

Required Education: Bachelor’s Degree from an accredited college or university in one of the following: Psychology, Social Work, Medicine, Nursing, Rehabilitation Counseling, Sociology, Human Growth and Development, Gerontology, Special Education, Educational Psychology, Early Childhood Education, Early Childhood Intervention, Child Development Therapy, Human Services, Human Development, Family Studies, or a related behavioral health field.

Requirements

• Valid Texas driver’s license with acceptable driving history
• Reliable transportation with proof of insurance
• Ability to pass criminal background screenings
• Ability to transport clients in personal vehicle
• Comfortable providing services in homes and community settings

Work Environment

This is a primarily field-based role requiring travel throughout the community and client homes. Candidates should be comfortable working in various community environments and supporting individuals experiencing mental health crises or significant life stressors.

Pay

$42,000.00 - $44,000 annually, plus bonus opportunities.
