Responsibilities

• Deliver strengths-based interventions tailored to your specific focus area (classrooms, early childhood settings, workplaces, or community spaces).
• Partner closely with clinicians, educators, employers, and treatment teams to ensure integrated, effective care plans.
• Link clients and families to essential wrap-around services, community programs, and local support systems.
• Provide trauma-informed support and de-escalation tailored to client and family needs in home and community settings.
• Maintain accurate, timely progress notes in compliance with state and agency standards.

Community-Based Case Management

Support youth, adults, and families in home and community settings to de-escalate crisis, foster healthy routines, and connect with vital local resources.
