Case Manager - Community-Based Services
About the role
CommQuest is seeking a Full-Time Case Manager to join our Canton Campus in Canton, Ohio. Case Managers (CM) provide services to individuals 18 and over, focusing on recovery and resiliency principles to empower them to integrate, function, and succeed in their natural environments. The CM assesses, plans, implements, coordinates, monitors, and evaluates the services needed to meet clients' needs. They also provide symptom monitoring, crisis management, advocacy, and connections to natural and community supports and resources.
Responsibilities
- Empowerment and promotion of recovery and resiliency principles.
- Facilitation, education, and development of sober living skills.
- Provides advocacy and outreach on behalf of clients.
- Provides client and family education demonstrating competency in clinical progress notes and supervisory observation.
- Limits clients with referral services, employers, family members, other providers, and any/all necessary resources to assist the client in obtaining goals.
- All other duties as assigned.
Requirements
- A Bachelor's degree in Social Work, Psychology, or a related Human Service field is highly preferred; minimum requirement of high school diploma or GED.
- Chemical Dependency Counseling Assistant (CDCA) through the Ohio Professionals Board, required.
- Previous experience working with adults with severe mental illness and substance use disorders, highly desired.
- Strong communication skills, both verbally and written, and the ability to work collaboratively with others on behalf of the client both within the agency and in the community.
- A valid driver's license, vehicle insurance, and a driving record that is in compliance with agency insurance requirements.
