Community Based Care Surveyor (Client Care Surveyor) Multiple Positions

About the role

The Oregon Department of Human Services is seeking Community-Based Care Surveyors (Client Care Surveyors) to ensure that individuals and their families receive person-centered services that promote safety, independence, and well-being.

Responsibilities

  • Conduct state licensing surveys according to state protocols, guidelines, and time frames.
  • Review facility survey history, complaint investigation reports, and data reports.
  • Observe details of residents’ quality of care and life, including staff assistance with activities of daily living, staff interactions with residents, nutrition, activities provided, and environment.
  • Interview residents, families and facility staff.
  • Review resident medical records and facility policies and procedures.
  • Evaluate conditions, using OARs, state guidelines, and individual professional judgment to validate and determine findings of potential non-compliance.
  • Write report of findings, using specified computer software program.

Requirements

  • Three years of experience in areas related to long-term care or nursing home administration.
  • One of the following:
    • A bachelor's degree in Health Science, Gerontology, Healthcare Administration, Long Term Care Administration.
    • ORA license as a Registered Nurse.
    • ORA license as a Long-Term Care Facility Administrator with a Bachelor’s in health services.
  • A valid driver’s license and acceptable driving record is required for this position.

Qualifications

  • Experience related to nursing in a long-term care setting or administration of a nursing care facility or assisted living.
  • Knowledge of Oregon State laws, rules, and regulations related to the provision of long-term direct-care programs and facilities.
  • Critical Thinking, investigation, and decision making: Skill in exercising independent judgment and decision-making in the application of laws, rules, policies, or procedures.
  • Teamwork, self-awareness, and growth mindset: Demonstrated ability to work successfully both independently and cohesively with a team.
  • Demonstrates emotional intelligence, conflict resolution skills, and unwavering professionalism—even in high-pressure or adversarial situations.
  • Experience using a variety of Microsoft Office Programs, email, data entry, video conferencing, and instant messaging.
  • Skilled in technical writing using the Principles of Documentation, with experience creating clear, accurate, and objective reports for medical, clinical, or long-term care investigations.
