Responsibilities
- Under RN direction and supervision, perform personal care, homemaker assistance, and defined nursing procedures for patients and their caregivers.
- Maintain competencies/contact hours as required by Company and regulations.
- Provide patient and caregiver services according to the Aide/Caregiver Plan of Care and Company policies and procedures.
- Avoid lifting patients and equipment, positioning, and transferring patients safely.
- Assist patients and caregivers in maintaining a clean and safe home.
- Provide caregiver relief as part of the Plan of Care.
- Transport supplies, lab specimens, documents, and other items needed to meet the Plan of Care as requested by the RN.
- Perform activities that promote activities of daily living under the direction or supervision of an RN.
- Contribute to the development of the Plan of Care by reporting observations to the RN.
- Communicate any significant issues or symptoms to the RN or Clinical Manager.
- During emergencies, assist in ensuring continuity of services.
- Perform other duties as assigned.
Qualifications
- High school diploma or GED or an equivalent combination of education and experience.
- Current certification as CNA in the state in which employee will be working.
- Minimum of six (6) months of CNA or Home Health Aide experience preferred.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- Valid driver’s license and automobile insurance per Company policy.
- Reliable transportation to meet work schedule.
- Ability to use equipment with visual and auditory mechanisms.
- Ability to communicate (written and verbal) in English.
- Ability to manage the emotional stress of working with and caring for ill patients and their families.
- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
- Ability to physically access any home or facility in assigned area (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning).
- Ability to be contacted by the company to report to work at a designated location during emergencies.
