About the role
The Per Diem RN plays a crucial role in providing outstanding patient care and high-level customer service in the Chapters Health Team. This position is more than just a job; it's a mission to support terminally ill patients and their families.
Responsibilities
- Achieve patient/family needs through the nursing process and coordinate with the Interdisciplinary Group (IDG).
- Provide palliative and supportive care to the patient/family unit.
- Complete clinical admissions, explaining services, addressing patient/family concerns, and obtaining informed consents.
- Initiate skilled nursing interventions, obtain physician orders, and document according to Company policy.
- Facilitate referrals to all service lines, work with referring hospitals, physicians, facilities, patients, families, and the general public.
- Communicate with after-hours staff, obtain consents for services, arrange safe discharges, and develop relationships with case managers, physicians, etc.
- Provide care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge.
- Manage the emotional stress of working with terminally ill patients and their families.
- Report changes in the patient’s condition to the IDG and other health professionals.
- Evaluate hospice referrals/admissions for level of care appropriateness.
- Prepare bodies for transport and oversee disposal of medications.
- Provide bereavement interventions to the family and caregiver.
- Present concise and pertinent oral and written reports to the IDG.
- Supervise patient care given by other nursing team members.
- Participate in triage during emergencies.
Qualifications
- Current license as RN in the state where the employee will be working.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- Minimum one (1) year nursing experience; hospice or hospital experience preferred.
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.
- Valid driver’s license and automobile insurance per Company policy.
- Reliable transportation to meet visit schedule.
- Ability to use equipment with visual and auditory mechanisms.
- Effective communication in English (verbal and written).
- Flexible schedule to meet the needs of Hospice House patients.
- Ability to manage emotional stress and physical demands of the job.
- Completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Schedule
Per Diem schedule will be determined based on patient needs.
