Core Competencies/Critical Responsibilities

  • Provides direct patient care to a select group of patients and advocates on their behalf.
  • Documents all activities in EMR according to defined guidelines.
  • Documents pertinent information, observations, and all aspects of patient care clearly, concisely, and in a timely manner.
  • Assists with ambulation and providing comfort items.
  • Performs bathing, dressing/undressing, bed pans/urinals (toileting), mouth care/grooming and shaving as directed by registered nurse.
  • Obtains vital signs, weights, empties/maintains drainage devices, measures intake and output, as directed by registered nurse.
  • Proactively rounds on patients at regular intervals to address needs and answer call bells.
  • Aids in meal distribution, prepping patient for procedure, changing bed linens, assisting with transfers, and managing supplies.
  • Maintains skin condition and reports to licensed nurse.
  • Assists with post-mortem care, restocking supplies, and managing multiple tasks.
  • Directs visitors as needed and manages priorities and allocation of available resources.
  • Ensures patient safety and quality of care through continuous communication with registered nurse.
  • Supports and promotes hospital-wide and unit-specific initiatives and goals.
  • Assists with patient education and care coordination.
  • Performs adjunct therapies as directed by registered nurse.
  • Performs phlebotomy, capillary blood glucose monitoring, EKGs, and wound/dressing changes.
  • Collects and records patient information, prepares treatment rooms, and schedules appointments.
  • Communicates with patients, families, and healthcare team to implement the plan of care.
  • Supports service recovery and maintains high standards of service.
  • Utilizes, navigates, and documents in an accurate and timely manner on the electronic medical record.
  • Maintains equipment and utilizes supplies in a safe, cost-effective manner.
  • Completes required continuous training and education, including department specific requirements.

Qualifications

  • Minimum of high school diploma, GED or equivalent.
  • Heart saver or equivalent required, or received within 30 days of hire.
  • Basic English language proficiency required.
  • Basic computer skills required.
