About the Role

Assess, plan, and coordinate support and services for children with physical, cognitive, and emotional disabilities. Support immediate family members and help problem-solve issues. This role significantly impacts children and families by ensuring they receive necessary services to remain safe in their homes and communities.

Responsibilities

  • Conduct assessments and develop Individual Service Plans for children in the Children’s Community Options Program (CCOP) and the Children’s Long-Term Support (CLTS) Medicaid Waivers Program.
  • Provide direct supportive services, including information/referral, supportive counseling, and assistance with financial aid applications.
  • Conduct home visits to discuss family concerns and document relevant information within CCOP and CLTS Medicaid Waivers Program.
  • Complete Wisconsin Children’s Long-Term Support Functional Screens (initial, annual, updates).
  • Handle paperwork such as case notes, letters, emails, faxes, funding requests/authorizations, Gainwell Technologies/Forward Health Authorizations, Individual Service Plans (ISP), outcomes, referrals, medical record reviews, Individual Education Plans (IEP) reviews, incident reports, data entry into the Forward Health Portal, high-cost funding requests, parental fee calculations, and other state/county forms.
  • Make home visits for intakes, reviews, check-ins, support, school meetings, and medical appointments.
  • Communicate with clients, providers, state contacts, therapists, evaluators, contractors, and other professionals via telephone.
  • Create and manage CLTS Waiver Third Party Administration (TPA) authorizations with Business Office staff.
  • Perform duties in compliance with Medicaid Home and Community-Based Services (HCBS) Waiver Manual and Medicaid Targeted Case Management regulations.
  • Manage CCOP fund expenditures, submit timely vouchers, and collaborate with Business Office staff on state-required reporting systems.
  • Adhere to Wood County safety policies and perform job tasks safely.
  • Embody Wood County Core Values and maintain courteous, respectful conduct.
  • Research items and services for clients.
  • Follow Wood County Human Services Department policies and comply with federal/state standards.
  • Participate in unit meetings and required training/conferences.
  • Maintain proper case files in compliance with department policy and regulations.
  • Monitor service costs for clients in CLTS and CCOP.
  • Perform other assigned duties.

Requirements

CLTS Support & Service Coordinator I – Uncertified

  • Associate’s degree in a human services-related field and at least two years of experience working with individuals/children with disabilities.
  • Knowledge of working with children with Developmental Disabilities (DD), Severe Emotional Disturbances (SED), and Physical Disabilities (PD).
  • In lieu of a degree, a combination of training in a health or human services-related field and experience in long-term care case management totaling four years (e.g., one year of coursework/training and three years of experience, or four years of experience in long-term support case management).

CLTS Support & Service Coordinator II – Certified

  • Bachelor of Arts/Bachelor of Science degree in a health or human services-related field and at least one year of professional experience working with individuals/children with disabilities.
  • Knowledge of working with children with DD, SED, and PD.

Additional Requirements for Both Roles

  • Knowledge of service delivery systems, integrated services, and resources.
  • Complete DHS’ Children’s Long-Term Support (CLTS) Program Training for Support and Service Coordinators (SSCs) and Mandated Reporter Online Training. Until training is completed, work under direct supervision.
  • Become certified to perform the Children’s Long-Term Support Functional Screen within one month of employment.
  • Possess basic everyday living skills, ability to understand/follow directions, read/write for reports, and interview/counsel/communicate effectively.
  • Travel to off-campus sites; valid driver’s license and insured automobile (or reliable transportation) required.
  • Operational familiarity with Microsoft Word, Excel, Streaming Healthcare Solutions software, and basic office equipment (computer/tablet, printers, telephone, copy/fax machine, calculator).

Pay

$54,505.75 to $66,595.75 annually.
