About the role
A member of the Children’s Long Term Support Care Management Team (CLTS) that is responsible for the coordination of case management services provided to a group of members. In addition, the CLTS Case Manager is responsible for locating, managing, coordinating and monitoring all proposed waiver services, other services and informal community supports needed by eligible persons and their families.
Responsibilities
- Coordinate case management services for assigned clients
- Locate, manage, coordinate and monitor waiver services, other services and informal community supports
- Ensure compliance with state and federal regulations
- Provide support and guidance to clients and their families
- Work collaboratively with other healthcare professionals and community resources
Qualifications
- Four-year bachelor’s degree in a social services area (e.g. social work, psychology, nursing, rehabilitation or behavioral disabilities or related field)
- At least two (2) years of experience working with children with Autism
- Knowledgeable of Wisconsin’s long-term support funding sources as well as the Medicaid Waiver programs
- Thorough understanding of community resources and long-term care options
- Valid Wisconsin driver’s license, reliable transportation and car insurance
