Clinical Therapist II (Full-Time)

About the role

As a Clinical Therapist II at Hackensack Meridian Health, you will treat patients using a variety of modalities as part of a multidisciplinary clinical treatment team. You will provide psychological and social interventions supporting patients and families in areas such as behavioral health, levels of care, hospitalization, disease, illness, wellness, and recovery.

Responsibilities

• Provides a comprehensive assessment of a patient's psychosocial needs, develops treatment plans, and conducts treatment plan reviews based on findings and department policy.
• Delivers effective behavioral health treatment and education to patients, including assessment, individual and/or group therapy, and shares information with staff, families, and approved agencies.
• Completes all documentation and written reports in a timely manner according to department and hospital policy.
• Provides services in accordance with licensure regulations defined by the respective licensure board.
• Utilizes and interprets information about patient needs, growth and development, and evidence-based treatment options to develop tailored treatment plans.
• Provides comprehensive case management services and assessments as warranted.
• Assesses Social Determinants of Health (SDOH) and provides information, education, and/or referrals to meet identified patient needs.
• Collaborates with the patient (with consent), family, and support persons to develop a comprehensive discharge plan addressing continuing care needs.
• Reassesses and revises treatment plans on an ongoing basis, evaluating progress and making clinical adjustments as needed.
• Maintains protected patient health information as required by law, state licensing, TJC, and HMH policy.
• Assists behavioral and medical team members in responding to emergencies.
• Refers patients to community agencies and resources as appropriate.
• Assists with the interview and orientation of new staff members.
• Supervises social work and counseling students completing field placements.
• Identifies and addresses the needs of diverse patient populations (e.g., age, culture, language, hearing or visual impairments) through tailored care and communication.
• Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs., and stands a minimum of 4 hours per day.
• Adheres to the standards identified in the Medical Center's Organizational Competencies.
• Receives proper supervision as required by the respective Licensed Board.
• Performs additional duties as assigned.

Qualifications

Education, Knowledge, Skills and Abilities Required:

• Master's degree in Social Work (MSW) or Master's degree in Counseling, or a related field.
• Minimum of two years of post-licensure work experience in the same field.
• Ability to perform psychosocial assessments and develop treatment plans.
• Knowledge of crisis intervention theory, disease management, disease process, differential diagnoses, and treatment techniques.
• Ability to work collaboratively as part of a team.
• Excellent follow-through and attention to detail.
• Strong written and verbal communication skills.
• Proficient computer skills (e.g., Microsoft Office, Google Suite).

Education, Knowledge, Skills and Abilities Preferred:

• Ability to provide guidance and mentorship to other team members.

Licenses and Certifications Required:

• Professional Counselor License, NJ Licensed Clinical Social Worker, or Marriage and Family Therapy license.
• EPS Department Specific Certification: Crisis Prevention & Intervention (CPI) certification, or completion required during new hire orientation.
• Carrier Clinic Specific Certification: Handle with Care certification, or completion required during new hire orientation.

Pay

Minimum starting rate of $86,569.60 annually. The posted rate is a reasonable good-faith estimate of the base pay for this role and does not reflect the full value of the total rewards package, which may include performance-based incentives, bonuses, or commissions. The final offer may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding.
