Clinical Therapist II (Full-Time)

This role is located in an outpatient department, and candidates with substance use experience are preferred.
Responsibilities
- Provides a comprehensive assessment of a patient's psychosocial needs, develops treatment plans, and conducts treatment plan reviews based on findings and according to department policy.
- Delivers effective behavioral health treatment and education to patients, including assessment, therapy (individual and/or group), and shares/interprets information with staff, families, and/or approved agencies.
- Completes all documentation and written reports in a timely fashion in accordance with department and hospital policy.
- Provides services to patients in accordance with licensure regulations defined by the respective licensure board.
- Utilizes and interprets information based on patient needs, growth and development, and evidence-based treatment options to develop tailored treatment plans for specific populations and age groups.
- Provides comprehensive case management services and assessments as warranted.
- Assesses Social Determinants of Health (SDOH) and provides information, education, and/or referrals to meet identified patient needs.
- Collaborates with the patient (with consent), family, and support persons to develop a comprehensive discharge plan addressing continuing care needs.
- Reassesses and revises treatment plans on an ongoing basis, evaluating progress and making clinical adjustments as needed.
- Maintains protected patient health information as required by law, state licensing, TJC, and HMH policy.
- Assists behavioral/medical team members in responding to emergencies and has working knowledge of community agencies/resources for appropriate referrals.
- Assists with the interview and orientation of new staff members.
- Supervises social work/counseling students completing field placements.
- Identifies and addresses the needs of diverse patient populations (e.g., age, culture, language, hearing/visual impairments) and provides care and instruction tailored to those needs.
- Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs., and stands a minimum of 4 hours a day.
- Adheres to the standards identified in the Medical Center's Organizational Competencies.
- Receives proper supervision as per their licensed board.
- Performs additional duties as assigned.
Requirements
- Master's degree in Social Work (MSW) or Master's degree in Counseling, or a related field.
- Minimum of two years of post-licensure work experience in the same field.
- Professional Counselor License, NJ Licensed Clinical Social Worker, or Marriage and Family Therapy license.
- Carrier Clinic Specific Certification: Handle with Care certification, or completion required during new hire orientation.
- Ability to perform psychosocial assessments and develop treatment plans.
- Knowledge of crisis intervention theory, disease management, disease process, differential diagnoses, and treatment techniques.
- Ability to work as part of a team and build collaborative relationships.
- Excellent follow-through and attention to detail.
- Strong written and verbal communication skills.
- Proficient computer skills (e.g., Microsoft Office, Google Suite).
Preferred: Ability to provide guidance/mentorship to other team members.

Pay
Minimum annual rate of $86,569.60. The starting rate of pay is provided for informational purposes and may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget/grant funding. Some positions may also be eligible for shift differentials, performance-based incentives, bonuses, or commissions.
