Clinical Team Manager - Catawba Team

Responsibilities

  • Manage day-to-day activities of an interdisciplinary team including supervision, resource management, scheduling, and assignments.
  • Ensure patient care is provided in accordance with organizational procedures and regulatory requirements.
  • Participate in recruitment, develop, supervise, and train clinical staff.
  • Provide professional development and coaching to ensure exceptional care and adherence to clinical documentation expectations.
  • Lead process improvement initiatives and actively monitor team performance.
  • Oversee interdisciplinary hospice team to ensure effective clinical care meeting patient and family needs.
  • Conduct timely performance appraisals and implement corrective actions as needed.
  • Assist HR with terminations in accordance with policies and job descriptions.
  • Support hiring, orientation, and mentoring of new employees.
  • Develop and revise plans of care through IDG meetings and other planning activities.
  • Ensure services comply with VIA’s structure, policies, and regulatory environment.
  • Base discharge decisions on meaningful assessments and appropriate procedures.
  • Ensure documentation meets all VIA standards and regulatory requirements.
  • Provide problem-solving and palliative care consultation to team members and peers.
  • Participate in quality improvement activities to monitor, evaluate, and improve team performance.
  • Ensure all clinical staff functions in accordance with established procedures and legal requirements.
  • Collaborate within EMR to complete recertification processes per organizational policies and regulations.
  • Provide back-office support within EMR as expected by the organization.
  • Administer Clinical Team functions effectively and review reports and statistical data to manage the team.
  • Monitor and improve patient care to meet professional, organizational, regulatory, licensure, and accreditation standards.
  • Stay current with healthcare trends and incorporate new information, policies, and procedures into practice.
  • Approve time sheets, payroll summaries, and PTO requests for direct reports.
  • Monitor compliance with internal initiatives and recommend policy modifications or new procedures based on practice patterns and needs.
  • Ensure continuity of patient care during staff absences.
  • Maintain professional relationships with providers, patients, families, and vendors.
  • Preserve and strengthen partnerships between hospice, long-term care, and physician practices.
  • Respond to customer concerns and facilitate prompt issue resolution.
  • Address problems and questions effectively and make practice changes based on feedback.
  • Respond to communications via text, email, and voicemail as necessary.
  • Present and maintain a highly professional image.
  • Establish and maintain effective working relationships with leadership and staff in other departments.
  • Attend meetings and participate actively in internal organizational activities; report and act on improvement opportunities.
  • Keep leadership informed of area activities and significant problems.
  • Cross-train with other Team Managers to ensure leadership continuity.
  • Clean patient rooms as required per cleaning procedures; maintain clean and stocked facility areas.
  • Keep family room clean throughout the day and use appropriate chemicals and protective equipment for cleaning.

Qualifications

  • Bachelor’s degree in a related field required; Master’s degree preferred, or equivalent combination of education, certifications, training, and work experience.
  • Minimum 2 years of related experience with increasing management responsibilities.
  • Related experience includes home health/hospice, oncology, and medical surgical.
  • Proficiency in Microsoft Office required.
