Clinical Support Nurse

About the role

Cordell Orthopaedics is seeking a Clinical Support Nurse to join our team in Concord, NH. This full-time, in-person position focuses on post-operative care management, patient education, care coordination, and clinical support.

Responsibilities

• Coordinating comprehensive pre-operative and post-operative patient care
• Performing post-operative care checks and supporting patients throughout their recovery
• Providing patient education on surgical procedures, recovery expectations, wound care, medications, and home care plans
• Aiding patients with care management needs, referrals, and discharge planning
• Assisting with patient education and support through the patient portal, phone, and email
• Developing relationships with rehabilitation facilities and monitoring patient progress
• Scheduling appropriate referrals and follow-up appointments
• Leading quality improvement initiatives and coordinating quality committee meetings
• Managing MIPS quality reporting and collaborating with department leaders to meet annual performance goals
• Collecting and reporting key Workers' Compensation metrics
• Leading specialty workgroups focused on standardizing patient care and improving outcomes
• Participating in professional organizations to identify best practices and enhance orthopedic patient care
• Performing other duties as assigned

Requirements

• Graduate of an accredited School of Nursing
• Current RN license in New Hampshire
• Current BLS Certification
• Excellent communication, customer service, telephone, and computer skills
• Ability to thrive in a fast-paced, collaborative healthcare environment

• Care management experience
• Post-operative surgical nursing experience
• Orthopedic nursing experience
• Experience coordinating care across multiple providers and healthcare settings
• Passion for patient education and improving the overall patient experience
