Responsibilities

• Reviews, processes, and modifies medication orders to maintain a current and accurate medication profile.
• Supervises, directs, and reviews activities involving compounding, storing, inventory, labeling, recording, or preparing medications by pharmacy technicians.
• Maintains drug therapies by evaluating appropriateness of use, dose, dosage form, regimen, route, therapeutic levels, drug interactions, and therapeutic duplication.
• Evaluates drug use compliance with prescribing criteria, guidelines, formulary, and protocols established by Norwegian American Hospital.
• Detects, monitors, documents, and reports adverse drug reactions and medication errors.
• Serves as a resource for drug information, including drug identification, side effects, adverse events, interactions, medication counseling, etc., for all practitioners and patients.
• Involves in QA/PI projects and initiatives to ensure compliance with state and federal regulations regarding controlled substances.
• Ensures proper control, documentation, and record keeping of controlled substances.

Requirements

• Graduate of an ACPE accredited college of pharmacy.
• Pharmacist licensure required.
