Clinical Staff Pharmacist

About the role

The Clinical Staff Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. The Clinical Staff Pharmacist proficiently provides direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel.

Responsibilities

• Properly assess patients' pharmaceutical needs and provide pharmaceutical care.
• Ensure accurate and proper compounding and dispensing of prescribed medications and solutions.
• Build and maintain partnerships with patients and other healthcare providers to improve patient outcomes.
• Counsel patients, patients' families, and healthcare professionals regarding drug therapy.
• Manage daily pharmacy workflow and operations in the absence of the pharmacist-in-charge or designee.
• Supervise and perform functions involved in the accurate and efficient dispensing of prescriptions.
• Review, fill, and verify medication orders for accuracy and completeness.
• Monitor patient drug therapy to detect and prevent drug interactions, contraindications, and adverse reactions.
• Screen all new medication orders for appropriate dose, drug interactions, allergies, and contraindications prior to dispensing.
• Provide counseling on all new medication orders to patients, families, and healthcare professionals.
• Make appropriate interventions to improve patient care based on current guideline-directed therapy.
• Use and operate the pharmacy computer system proficiently.
• Compound and dispense medications and other pharmaceutical agents/products.
• Obtain pertinent patient medication and medical information.
• Provide current and accurate drug information to patients and healthcare professionals.
• Communicate effectively with patients, healthcare professionals, and coworkers.
• Serve on hospital and system committees as assigned.
• Complete and properly document medication therapy management interventions.
• Ensure compliance with company and legal policies, procedures, and regulations.
• Ensure confidentiality of information, documentation, and records.
• Administer vaccines and other injections (Retail).
• Maintain and direct pharmacy operational workflow in the absence of the retail pharmacy manager (Retail).
• Dispense and/or administer medications to patients in accordance with policies, including immunizations, naloxone, and injectable medications where permitted by law (Retail).
• Supervise and perform functions involved in the I.V. Admixture Program, including preparation and delivery of total parenteral nutrition solutions, chemotherapy, antibiotics, and other medications for intravenous/irrigation therapy (Hospital).

Qualifications

• Bachelor of Science degree in Pharmacy or Pharm D.
• Current state pharmacist licensure in good standing or licensure eligibility in state of employment within 90 days of hire.
• Immunization certification (if Retail or Specialty).
• Preferred: 1-2 years of pharmacy experience.
• Maintain training and education needed to provide patient-centric pharmacy services.
• Retail: Obtain and maintain training and education for comprehensive pharmacy services, including naloxone, immunization certification, medication therapy management, and state and federal laws and regulations.
