About the Role
This is a temporary, benefitted opportunity for a pharmacist who wants to take a more focused, patient-centered approach to care. In this role, you’ll assume primary responsibility for medication management for a dedicated group of patients. - Review and monitor medication therapies to ensure safety and effectiveness.
- Collaborate with physicians, nurses, and care teams on treatment plans.
- Provide recommendations to optimize therapy, including cost-effective options when appropriate.
- Counsel patients on how to use their medications safely and confidently.
- Document and track medication-related interventions and outcomes.

Qualifications
- Required Education and Experience: Doctor of Pharmacy degree; or Master’s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; or Bachelor’s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience.
- License Requirement: License to practice as a Registered Pharmacist in the State of New York.
- Preferred Qualifications: At least 1 of the following: Completed PGY1 Pharmacy Practice Residency Board certified In the process of completion of PGY2 Pharmacy Specialty Residency ProgramExperience in all 3 of the following:1. 3+ years of work experience in a position of Clinical Pharmacist Specialist (PGY1 Pharmacy Residency accounted for 3 years of clinical experienceExperience in of the following: Contributions to scholar activities (within the last 3 years) such as research, publications, and poster presentation. Experience in developing programs contributing to pharmacy practice and patient safety and experience.
