Responsibilities
- Participate and assist in the direction, functional supervision and provision of daily home infusion pharmacy operations.
- Pharmaceutical patient assessment, care planning and disease state management.
- Clinical drug monitoring.
- Patient and clinician follow up.
- Care coordination with other health care professionals.
- Computer data entry, production, packaging, shipping, quality assurance, and regulatory compliance.
Required Qualifications
- Doctor of Pharmacy degree or Master and/or Bachelor of Science degree in Pharmacy with one year residency or fellowship or equivalent clinical experience.
- A Certificate of Residency from an accredited program or an equivalent combination of education and experience is necessary.
- Ability to read, write, speak, and comprehend English.
- Some experience in clinical pharmacy is necessary.
- Some experience in didactic and clinical teaching, including undergraduate and postgraduate level courses, is necessary.
- Licensure in good standing, or immediate eligibility for Licensure, as a Registered Pharmacist with an active controlled substance license in the State of Michigan.
- Possession of a state narcotics license is necessary.
Desired Qualifications
- Experience in home infusion pharmacy with a demonstrated knowledge of regulations, standards, and guidelines applicable to home infusion pharmacies, admixture practices, and clinical pharmacy services.
Physical Requirements
- Functional visual acuity and hearing for accurate assessment and recording of patient observations and information in clinical records.
- Able to perform the assigned duties and responsibilities with or without reasonable accommodations: ability to work in a sedentary position for long periods of time, ability to communicate/ exchange information, fine motor coordination/dexterity necessary for keyboarding or typing, and may require reaching, bending, twisting, stooping, moving about, lifting of up to 10 pounds, etc., with or without the use of assistive/adaptive devices.
