Clinical Nurse - Transitional Care Unit

Key Responsibilities

• Provide direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within nursing standards.
• Educate patients and families on managing illnesses, post-treatment care, and medication administration.
• Collaborate with a multidisciplinary team to develop, implement, and document individualized care plans.
• Assess physical, psychosocial, and emotional needs, respecting patient preferences and cultural values.
• Participate in activities promoting patient safety, quality, and regulatory compliance, as well as professional development.

Qualifications & Skills

• Required: Nursing Diploma/Associate's Degree and RN licensure.
• Preferred: Bachelor's Degree in Nursing and two years of experience.
• Strong critical thinking, assessment, and clinical decision-making skills.
• Ability to work in a fast-paced, dynamic environment and collaborate with healthcare professionals.
