About the role

The Registered Nurse in the Transitional Care Unit provides direct patient care, including assessment, planning, implementation, and evaluation within nursing standards. They promote patient and family-centered care, educate on illness management, and participate in safety and quality activities. Collaboration with multidisciplinary teams is essential to ensure comprehensive care and effective communication of care plans. The role involves assessing physical, psychosocial, and emotional needs, respecting patient preferences and cultural considerations.

Responsibilities

Qualifications

• Associate's or Bachelor's degree in Nursing.
• Current RN licensure.
• Prior experience is preferred but not required; 2+ years is ideal for advanced roles.
• Strong assessment, critical thinking, and communication skills.
• Ability to work collaboratively within a multidisciplinary team.

Schedule

Work two twelve-hour night shifts per week.
