About the Role
As an Clinical Nurse Liaison at Aspire Senior Living, you will play a critical role in fostering effective communication between our nursing staff and medical professionals. Your expertise will help ensure seamless transitions, accurate documentation, and dignified care for our residents.
Responsibilities
- Coordinating with medical staff to facilitate smooth care transitions, including medication management, test results, and treatment plans
- Providing accurate and timely communication to residents, family members, and healthcare providers regarding resident care and status
- Collaborating with our nursing staff to develop and implement individualized care plans that meet the unique needs of each resident
- Maintaining accurate and up-to-date records, including medication administration records and progress notes

Requirements
- Current LPN/RN licensure in the state of Missouri
- Minimum 1 year of experience in a long-term care setting
- Ability to work effectively in a fast-paced environment with multiple priorities
- Excellent communication, interpersonal, and problem-solving skills
