About the role

The Clinical Liaison Nurse performs clinical assessments of patients for home infusion therapy and develops strategies to capture referrals from hospital systems. They act as a primary relationship builder between the organization and hospital staff to ensure quality coordination of care.

This is a Per Diem position in Bangor, Maine, allowing you to choose your schedule. Clinical Liaison Nurses in this role typically work 1-3 days a week from 8:00 am until 5:00 pm.

Responsibilities

  • Gathers and communicates to NELC’s intake department demographic, clinical, and reimbursement information on patients referred to NELC.
  • Develops relationships and seeks opportunities to bring all potential home infusion referrals to NELC, including within hospitals, ambulatory care providers, and member hospital care providers (e.g., urgent care, SNF).
  • Communicates results of insurance verification to hospital discharge planners and/or patients. Assists the NELC reimbursement department in exploring all possible sources of reimbursement.
  • Reviews needs and abilities of the patient and caregivers. Participates in developing a patient-specific training plan and plan of care, including patient training when appropriate.
  • Facilitates the discharge or referral process by relaying orders and documentation to the appropriate NELC departments.
  • Acts as clinical liaison between NELC and all appropriate hospital departments, including medical staff, care coordination/case management, social services, pharmacy, and nursing.
  • Assists in coordinating services with other home health agencies.
  • Monitors active and pending NELC patient status while hospitalized and provides ongoing communication regarding patient status to appropriate NELC staff.
  • Maintains an appropriate patient caseload as defined by the Director of Nursing.
  • Comprehensively documents all interactions with patients and healthcare team members in the patient record and/or electronic progress notes.
  • Effectively communicates patient requirements and changes in patient status to NELC infusion and reimbursement teams, as well as Home Health Agency (HHA) staff.
  • Participates in daily patient care meetings and case conferences.
  • Participates as a member of the hospital’s care management team in hospital rounds to identify and provide care coordination and guidance to the hospital care team, providers, and patients referred for infusion.
  • Ensures confidentiality of patient information in accordance with state and federal HIPAA Privacy and Security regulations.
  • Accurately completes and determines reimbursement for therapy-specific checklists (e.g., Medicare TPN, Medicare ENT, Medicare Cardiac) as required by payer or NELC guidelines.

Requirements

  • Associate degree or diploma in Nursing.
  • Two (2) years of intravenous therapy experience in a hospital or homecare setting.
  • Excellent patient assessment skills.
  • RN/LPN license in good standing in the state(s) where providing care/services.
  • Valid driver’s license.
  • Excellent driving record.

Preferred Qualifications

  • BS in Nursing.
  • Current BLS Certification.
  • Knowledge of HIPAA Privacy and Security Regulations.
  • Knowledge of JCAHO accreditation process and standards.
  • Working/practical knowledge of home infusion therapy administration and equipment use.

Pay

$50/hour

Schedule

Per Diem, with flexibility to choose 1-3 days per week (8:00 am – 5:00 pm).
