POSITION SUMMARY

Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transition for the patient from the hospital to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.

ESSENTIAL FUNCTIONS

  • Respond to referral sources in a timely manner;
  • Evaluate patient appropriateness for hospice;
  • Meet with families/patients to discuss goals and explain services of hospice.
  • Complete required paperwork with patients/families;
  • Facilitate the patient discharge by working with hospital staff, DME, medications, hospice physicians, and hospital staff.
  • Educate hospital staff and follow up on pending patients;
  • Round daily to be available to staff and patients for questions and to build relationships.
  • Communicate and provide daily feedback to the admission staff on referrals.
  • Completes thorough documentation of hospice eligibility in the required time frame.
  • Through customer service and leveraging relationships, become the “hospice of choice” whereby increasing hospital referrals.
  • Attend and participate in routine staff meetings in person or via media provided as directed.
  • Participate in and/or provide in-service training, attend various hospital activities, and perform other duties as required or assigned.
