Clinical Nurse Coordinator, Supportive Care

About the role

The Clinical Nurse Coordinator (CNC) provides clinical support to medical providers, offering care management for their entire patient panel. As the primary point of contact for complex outpatients, the CNC ensures continuity of care through nursing assessments, triage, patient education, and care coordination. This role is an integral part of a dedicated multidisciplinary team collaborating to deliver the highest level of patient care.

Responsibilities

• Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes.
• Develops a plan of care based on patients’ disease, symptoms, and response to treatment.
• Educates patients, families, and caregivers on disease processes, treatment, side effects, and symptom management.
• Coordinates with the interdisciplinary team to ensure continuity of care.
• Refers patients, families, and caregivers to other clinical resources/professionals such as social work, home health care, and clinical nutrition.
• Implements, evaluates, and modifies the plan of care.
• Integrates research and evidence-based knowledge into clinical practice.

Direct Patient Care

• Patient and family education.
• Telephone and email symptom triage.
• Medication administration and injections.
• New patient orientation.
• Caregiver support.
• Assessment of needs for additional supportive care resources.
• Prescription refills.

Indirect Patient Care

• Supportive care referrals.
• Coordination of outside clinical care.
• Coordination of Hospice and Home Health.
• Ensuring continuity of care with outside providers.
• Collaboration with the research team for patients on trial.
• Coordination with other oncology teams.

Communication and Teamwork

• Attends regular team meetings with providers and nursing teams.
• Coordinates care with ancillary services.
• Delegates tasks to appropriate team members.
• Coordinates patient schedules with the clinic team.
• Collaborates with other care team members to meet patient needs.
• Manages and escalates issues in clinic in real-time.

Professional Development and Clinic Engagement

• Documents all nursing care activities accurately and in a timely fashion.
• Demonstrates knowledge and understanding of patient privacy rights.
• Actively participates in continuing education, departmental projects, committee work, and quality improvement initiatives.

Requirements

• Graduate from an accredited school of nursing.
• Current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); MSL primary state of residence must be updated to WA within 60 days of hire (may be waived under specific circumstances).
• Current BLS AHA Health Provider Card (or equivalent) with renewal required every two years.
• Additional certification may be required dependent upon department.
• Critical thinking skills, including analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge.
• Ability to develop clinical judgment.
• Time management skills; ability to organize and manage time and tasks independently.
• Effective communication skills, including active listening, reflecting, acknowledging, summarizing, empathizing, and paraphrasing.
• Ability to provide information clearly by checking prior knowledge, giving small amounts of information at a time, using clear terms, avoiding jargon, and verifying understanding.

Qualifications

Preferred:

• BSN-prepared nurse.
• OCN (Oncology Certified Nurse) or BMT RN certification.
• 2 years of oncology nursing experience.
• Previous telephone triage experience.
• Previous outpatient clinic experience.
• Initiative-taking, strong communication, multitasking, and organizational skills.

Pay

The hourly pay range for this position is from $47.64 to $88.19, with pay based on experience and qualifications. Registered Nurses are paid on a step-scale basis and may be eligible for shift differentials and other pay premiums such as advanced degree, preceptor, and certification pay. This position may also be eligible for relocation assistance.

Schedule

This position is 0.9 FTE, 36 hours per week.
