Responsibilities
- Recommend employment decisions (hire, fire, promote, change status) to the director of operations.
- Ensure overall quality of patient care and clinical performance of nurses and aides.
- Organize clinical operations for the care center.
- Review service requests and determine patient eligibility/suitability for home care.
- Coordinate patient referrals and admissions with intake.
- Oversee scheduling, payor verification, and precertification requirements.
- Review staff documentation to resolve missing, incomplete, or untimely issues.
- Assist clinicians in establishing patient goals, priorities, and developing the Plan of Care (POC).
- Regularly evaluate patient needs and revise the plan of care in collaboration with clinicians.
- Provide direct patient care infrequently during emergencies.
- Participate in the on-call process.
- Train staff to ensure quality care, compliance, and fiscal responsibility.
- Investigate and resolve patient, family, or physician concerns.
Requirements
- Associate or bachelor’s degree in nursing.
- Current RN license specific to the state(s) of assignment.
- One year of clinical RN experience in home health or hospice.
- One year of RN management experience.
- Current CPR certification.

Pay
$86,800 to $105,000 annually.
