About the role
Encore Healthcare is seeking a Clinical Liaison to serve our three communities: Encore Wilmington, Encore Foulk, and Encore Parkside. This role is responsible for clinically assessing patients as part of the admissions process to determine eligibility, utilizing on-site visits, medical records, and professional judgment. The Clinical Liaison also oversees all marketing efforts for the facility, including developing and maintaining relationships with external referral sources.
Responsibilities
- Generate new referrals by developing and maintaining relationships with hospital case managers, social workers, physicians, and community referral sources.
- Develop and execute a comprehensive sales and marketing strategy to increase occupancy and revenue across our three communities.
- Lead patient eligibility assessments for Skilled Nursing and Long-Term Care through in-person evaluations, medical record reviews, medication list analysis, and coordination with the facility admissions department.
- Complete written assessments and provide proper documentation to Encore staff.
- Respond promptly and thoroughly to incoming referrals.
- Communicate with patients and their families, and conduct facility tours.
- Coordinate with community staff to ensure smooth transitions through the admission process.
- Demonstrate knowledge of health insurance coverage and obtain authorizations for Skilled Nursing admissions.
- Utilize the EMR system and follow the admission notification process.
- Schedule and participate in speaking engagements at community and professional events.
- Contribute to the facility’s social media presence.
- Collaborate with Encore’s Admissions, Sales, and Marketing teams to promote the facility’s strengths and services.
- Maintain professional, positive, and appropriate behavior as a representative of Encore.
- Adhere to all admissions, marketing policies, and established procedures, including communication and documentation tools.
- Attend necessary community meetings.
- Prepare daily and weekly Marketing Reports by tracking referrals and analyzing data and trends.
- Participate in daily/weekly conference calls with the VP of Business Development.
- Work collaboratively as part of the admissions/marketing team, including covering for peers in the admissions office.
Requirements
- Minimum of 1 year of experience in a Skilled Nursing healthcare setting.
- Ability to professionally navigate various settings, including hospitals, skilled nursing facilities, independent and assisted living communities, private homes, and other environments as needed.
- Must be able to work flexible hours to accommodate facility needs.
