Clinical Liaison
Position Responsibility
Manages daily and weekly schedule of patient care transition coordination, patient assessment and teaching, and marketing support activities.
Provides patient education teaching sessions in the facility.
Maintains effective working relationships with facility referral sources.
Sets out new facility referral source contacts and opportunities for additional referral activity from existing referral sources.
Collaborates with the Account Executives in the market for business development activities.
Completes clinical and patient care coordination documents timely and accurately.
Aids Nurse Manager or General Manager with education for Home Health Agency staff.
Demonstrates positive, informative interaction with other departments, co-workers, referral sources and hospital physicians.
Adheres to all applicable policies, procedures, safety plans and standards imposed by regulatory organizations and Vital Care.
Captures referral source and patient activities closely with the Account Executive(s) responsible for the customer account.
Qualification
- A minimum of two (2) years of previous nursing experience, with case management or home health preferred
- Previous marketing experience preferred
- Excellent interpersonal and customer service skills
Physical Requirements
- Daily driving throughout assigned territory.
- Ability to travel up to 10% of time.
- While performing the duties of this job, the employee is regularly required to sit and talk or listen.
- The employee is frequently required to walk and use hands to finger, handle or feel.
- The employee is occasionally required to lift and/or move up to 10 pounds and occasionally lift and or move up to 30 pounds.
- Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus.
- The noise level in the work environment is usually moderate.
