About the role
We’re seeking an experienced Clinical Liaison to join our collaborative, patient-focused team. This role plays a vital part in strengthening clinical partnerships, supporting our referral network, and ensuring seamless transitions for patients into inpatient rehabilitation care.
Requirements
- Clinically licensed professional (RN, LVN, PT, OT, ST, LMSW, or RT)
- Previous IRF (Inpatient Rehabilitation Facility) experience required
- Strong understanding of inpatient rehabilitation
- Experience completing Pre-Admission Screening (PAS)
- Relationship-driven, adaptable, and clinically confident
- Comfortable working in a dynamic, evolving market
Responsibilities
- Serve as a key clinical resource within our community, fostering relationships with referral partners, case managers, physicians, and healthcare professionals.
- Complete thorough pre-admission assessments, review medical records, interview patients and families, and determine clinical appropriateness for admission into our IRF (Inpatient Rehabilitation Facility).
- Ensure compliance with CMS/IRF regulations and consistently communicate admission status with internal teams and external partners.
- Provide education and support to referral sources, patients, and their families regarding services, care expectations, and rehabilitation pathways.
- Represent Acute Rehabilitation Hospital of Plano at community events, educational opportunities, and professional networks.
- Collaborate closely with Business Development, Admissions, and Clinical teams to support operational excellence and high-quality patient care.
