About the role
We are seeking a motivated and relationship-driven Post Acute Liaison to serve as the primary connection between hospitals, physicians, case managers, and post-acute care services. This role is responsible for developing referral relationships, coordinating transitions of care, and helping patients access appropriate post-acute services in a timely and compassionate manner.
Responsibilities
- Develop referral relationships with hospitals, physicians, case managers, and post-acute care services.
- Carefully coordinate transitions of care to ensure patients receive appropriate post-acute services.
- Help patients access appropriate post-acute services in a timely and compassionate manner.
Requirements
- Current/prior experience/license as an RN, LPN, LCSW, Therapist (PT, OP, or SP), or Marketing Specialist (highly desirable).
- Bachelor's degree or MPH preferred.
- Two or more years in a related field that demonstrates the ability to work in a long-term care setting, preferably with geriatric resident admissions.
- MEDICAL/BUSINESS OFFICE EDUCATION OR EXPERIENCE PREFERRED.
Qualifications
- Strong healthcare relationship-building skills.
- Knowledge of discharge planning and care transitions.
- Experience working with hospitals, skilled nursing facilities, home health, hospice, rehab, or long-term care services.
Skills
- Excellent communication and interpersonal skills.
- Ability to manage multiple tasks and prioritize effectively.
- Strong organizational and problem-solving skills.
Pay
Annual Salary Range: $75,000 - $85,000 (DOE) plus Commission.
Schedule
Full-time position.
