Post-Acute Liaison
About the Role
The Post-Acute Liaison (also known as Admissions Director or Clinical Liaison) generates business from referral sources to meet the community’s revenue and census goals. This position primarily involves outside sales, developing relationships with community hospitals, physician offices, community health clinics, and other outreach programs. The role contributes to HumanGood’s success by coordinating external professional relationships, emphasizing Skilled Nursing components to achieve the highest possible occupancy within budgetary and regulatory guidelines.
Responsibilities
- Develop and maintain relationships with referral sources such as hospitals, physician offices, and community health clinics.
- Drive business to meet revenue and census goals for the Skilled Nursing team.
- Coordinate external professional relationships to maximize occupancy within budgetary and regulatory guidelines.
Requirements
- Two or more years of experience in a related field, preferably in a long-term care setting with geriatric residents, admissions, or marketing.
- Current or prior experience as an Admissions, Clinical, or Post-Acute Liaison within a skilled nursing facility preferred.
- Clinical license such as RN, LPN/LVN, RT, IT, or SLP preferred.
- Bachelor’s Degree, or background as a Therapist (PT, OT, or SLP), LCSW, Nurse, MPH, or Marketing Specialist preferred.
- Valid driver’s license.
- Medical or business office education or training desired.
Pay
Annual salary range: $65,000 to $85,000 (DOE) plus commission.
