About the role
As a Post Acute Alignment Specialist, you will serve as the vital link between hospital teams and Banner’s specialty services. You will spend 80% of your time on-site and 20% engaging with select valley clinics, participating in daily physician and case management rounds to facilitate referrals for specialty pharmacy, home medical equipment, and home infusion therapy. This role ensures patients receive the right care at the right time by collaborating with providers, care coordination staff, operations support specialists, and business development teams. You will also educate physicians and their staff on Banner’s comprehensive specialty, HME, and home infusion products and services.
Responsibilities
- Develop and maintain contact with key hospital, skilled nursing, assisted living facility, discharge planning services, case management, and clinical staff to provide ongoing updates on Banner Homecare services available in the market.
- Increase admissions within assigned accounts and meet or exceed targets.
- Develop a Homecare presence within facilities.
- Provide education and/or continuing education programs for referral sources within guidelines of facility and company policy.
- Utilize appropriate company resources to maintain and develop business opportunities.
- Maintain market awareness and prepare competitive updates as directed.
- Maintain positive relationships with referral sources and facilitate communication between referral sources, patients, and family populations within assigned accounts.
- Attend professional organization meetings as approved.
- Maintain and follow patients while in the facility, functioning as a liaison between referral sources and Banner Homecare.
- Assess needs of patient/family, ensuring clinical alignment is appropriate and securing appropriate post-acute resources.
- Ensure effective communication and collaboration with program staff and actively participate in weekly program business development meetings.
- Bring relevant data, reports, and information regarding changes within accounts and referral sources.
- Assist with new staff orientation and physician orientation programs as directed.
- Demonstrate competent professional practice and engage in opportunities for professional growth.
- Identify opportunities for and participate in performance improvement activities.
- Work independently under general supervision.
Requirements
- Strong knowledge of clinical care, typically obtained through a degree in nursing, healthcare, or a related field.
- Three or more years of related sales experience and education in healthcare services sales and marketing.
- High degree of organization, time management, and ability to work independently to achieve goals.
- Effective negotiation, conflict management, customer service, and interpersonal relations skills.
- Strong verbal, written, and computer communication skills.
- Must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it for the duration of employment.
Preferred Qualifications
- Home health and/or hospice experience.
- Additional related education and/or experience.
