About the role
Bridges Health is seeking a Part-Time Weekend Post-Acute Liaison (hospital liaison RN/LPN) to join our Administration department in Oklahoma City, OK. This is a non‑exempt, remote position (assessments/transfer only, no hospital visits) with hiring limited to weekends. Being located in the Oklahoma City or Tulsa metro area is highly preferred. Responsibilities
- Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs.
- Develop and maintain strong referral development relationship with referral sources.
- Collaborate with facility Admissions Coordinators to ensure a smooth, patient-centered admission experience.
- Maintain accurate records of daily activity and referral source interactions within the referral tracking system.
Requirements
- Current and unencumbered Oklahoma LPN or RN license
- Experience as a hospital liaison
- Experience in a skilled nursing or skilled rehabilitation setting
