Position Summary

The Clinical Coordinator, Licensed Practical Nurse will collaborate and coordinate with the Clinical Coordinator, Registered Nurse(s) and Clinical Managers to support the overall clinical operations, functions, and activities of the Home Care team. This position assists with the coordination of clinical operations and documentation processes to support high-quality, compliant home care services. This role utilizes clinical knowledge and critical thinking skills to manage patient supply ordering and assist with quality assurance processes within the EMR, coordinate Start of Care (SOC) activities, and collaborate with interdisciplinary teams to ensure timely and effective patient care. The Clinical Coordinator, Licensed Practical Nurse also supports agency performance improvement initiatives through QAPI audits, patient care coordination, and ongoing communication with physicians, staff, patients, and community resources.

About the Role

Education: Graduate of an accredited school of practical nursing. Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing) required. Current and active Basic Life Support (BLS) certification required.

Responsibilities

  • Collaborate with Clinical Coordinator, Registered Nurse(s) and Clinical Managers to support clinical operations and documentation processes.
  • Manage patient supply ordering and assist with quality assurance processes within the EMR.
  • Coordinate Start of Care (SOC) activities.
  • Collaborate with interdisciplinary teams to ensure timely and effective patient care.
  • Support agency performance improvement initiatives through QAPI audits, patient care coordination, and ongoing communication with physicians, staff, patients, and community resources.

Requirements

  • Minimum of one year of experience as a Licensed Practical Nurse in home care or community health preferred.
  • Supervisory experience preferred.

  • Demonstrates effective interaction and communication (oral, writing, presenting) skills.
  • Effective organization skills and ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure.
  • Ability to function competently in stressful situations and a changing work environment related to changing patient needs, including working with patients with acute, chronic, and complex disease processes, and those who are dying.
  • Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, Lab, etc.).
  • Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations.
  • Ability to use critical thinking.
