Qualifications

• Required:
  • RN graduate of an accredited school of nursing
  • Minimum of 2 years clinical experience in an acute care setting
  • NJ State Professional Registered Nurse license
• Preferred:
  • BSN
  • Transfer center experience
  • Critical care and emergency medical services experience

Job Duties

• Facilitates communication between sending and receiving facilities, including the use of telephone, computer software, and other communication and documentation tools.
• Determines and documents a record of the encounter.
• Utilizes clinical expertise, critical thinking, and demonstrated knowledge of unit-specific admission and discharge criteria.
• Answers telephones in accordance with established policies and procedures.
• Utilizes professional tone and demeanor in all communication.
• Coordinates communication between sending and receiving facilities, and within the receiving facility.
• Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner.
• Works in a collaborative manner with other members of the health care team.
• Maintains awareness of bed availability on all patient care units and outpatient departments involved in the inter-facility transfer and direct admission of patients.
• Escalates barriers and concerns using applicable policies and procedures as well as chain of command.
• Follows patient identification policies and procedures to ensure accurate patient identification.
• Mentors team members.
• Participates in onboarding of new team members.
• Maintains a clean and sanitary work environment.
