About the role

The Hub Clinical Care Coordinator facilitates transfers and acceptance of patients for all facilities within Tanner Health System. This position collects clinical information related to transfer requests and matches patients' clinical needs to facilities with bed capacity and service capability. The role involves collaboration with multiple departments, referring facilities, physicians, and hospital staff.

Requirements

• Associate Degree or 2 years of college coursework.
• Three years of related experience.
• Working knowledge of specialized practices, equipment, and procedures.
• Georgia Registered Nurse license or multistate nursing license.
